Section 5(a)(1) of the Occupational Safety and Health Act of 1970: The employer did not furnish employment and a place of employment which were free from recognized hazards that were causing or likely to cause death or serious physical harm to employees in that employees were exposed to: a) ...the hazard of being struck by an overhead valve wheel. At Building #123 on the 2nd floor, employees were exposed to an unchained overhead isolation valve wheel that could be spun off and fall, striking an employee. Among other methods, one feasible and acceptable means of abatement is to: 1. Install a safety chain to retain the wheel if it came loose. b) ...the hazard of being struck by an overhead valve wheel. At Building #2 on the 1st floor, employees were exposed to an unchained overhead isolation valve wheel that could be spun off and fall, striking an employee. Among other methods, one feasible and acceptable means of abatement is to: 1. Install a safety chain to retain the wheel if it came loose. c) ...the hazard of being struck by an overhead valve wheel. At Building #24 on the 1st floor, employees were exposed to an unchained overhead isolation valve wheels that could be spun off and fall, striking an employee. At Building #4 on the top floor, employees were exposed to an unchained overhead isolation valve wheel that could be spun off and fall, striking an employee. At Building #87 on the 4th floor (north side), employees were exposed to an unchained overhead isolation valve wheels that could be spun off and fall, striking an employee. Among other methods, one feasible and acceptable means of abatement is to: 1. Install a safety chain to retain the wheel if it came loose. DISCLAIMERS: 1.
