29 CFR 1910.333(c)(2): The employer did not ensure that only qualified persons were working on energized circuits and that qualified persons were familiar with the proper use of special precautionary techniques, personal protective equipment, insulating and shielding materials, and insulated tools: a.) American Refining Group, Inc. - On or about January 28, 2015, and at times prior thereto, qualified persons were not familiar with the proper use of electrically related personal protective equipment during installation of an Allen Bradley brand 480 Volt starter. The 480V starter was being installed into the Crude Unit Motor Control Center which was not de-energized prior to installation of the motor starter.
