29 CFR 1926.503(a)(2)(ii): The employer did not assure that each employee, as necessary, had been trained by a competent person in the correct procedures for erecting, maintaining, disassembling and inspecting the fall protection systems to be used: a. Site- Employees directing the installation of wood guardrail systems were not provided training in the correct procedures for installing wood guardrail systems. On or about January 12, 2015 an employee sustained severe injuries after falling from the second floor at the ladder way where the guardrail system failed when the employee was leaning against the top rail. The guardrail system built around the perimeter of the floor was constructed using support posts spaced 12 feet on center and the support posts on either side of the ladder way were not framed in two directions to provide strength to the termination points of the guardrail system.
