29 CFR 1910.27(c)(4): The distance from the centerline of rungs, cleats, or steps to the nearest permanent object in back of the ladder was less than 7 inches: Employees were exposed to a fall hazard at the Unit 6 Grade Floor by the 62 Boiler because: a. The distance between the centerline of the bottom rung to the steel structure was approximately one inch on the fixed ladder located on the pedestal at 622 GR Fan at 470' elevation and up to the platform located at 483' elevation located above the 621 Economizer Tank. b. The distance between the centerline of the second rung from the bottom to the pipe was approximately 3-1/4 inches on the fixed ladder located on the pedestal at 622 GR Fan at 470' elevation and up to the platform located at 483' elevation located above 621 Economizer Tank. c. The distance between the centerline of the third rung from the bottom to the steel/concrete structure was approximately one inch on the fixed ladder located on the pedestal at 622 GR Fan at 470' elevation and up to the platform located at 483' elevation located above the 621 Economizer Tank.
