29 CFR 1926.452(w)(5): Scaffolds shall be stabilized to prevent tipping during movement: Dusit Thani Hotel Jobsite, Sixth Floor, Banquet Room 1 and 2: Pacific Rim Land Development permitted a four tier fabricated frame orange mobile scaffold to be moved by employees; the mobile scaffold was not stabilized with outriggers or similar stabilizing devices. On October 18, 2014, the mobile scaffold toppled over fatally injuring a plumber. The scaffold was hazardous in that the scaffold frame legs were not designed with holes to accept mobile scaffold wheel stem pins to prevent the wheels from coming out of the frame legs; and the height to base ratio of the mobile scaffold was greater than 4 to 1. When the orange scaffold toppled it was measured to be 16 feet, 7.5 inches tall and 4 feet wide.
