29 CFR 1910.215(a)(4): On offhand grinding machines, work rests were not used to support the work and/or were not kept adjusted closely to the wheel with a maximum opening of one-eighth inch to prevent the work from being jammed between the wheel and the rest, which may cause wheel breakage. (a) Northeast Corner of Shop - On or about September 18, 2014 employees were exposed to struck-by hazards while using a K and F Bench Grinder (serial no. 9511789). Bench grinder did not have a work rest to support the work.
