29 CFR 1926.502(i)(3): All covers were not secured when installed so as to prevent accidental displacement by the wind, equipment, or employees: (a) An OSB sheathing that was used to cover a roof vent shaft opening that was approximately 69-inches by 42-inches was not secured to prevent accidental displacement. The vent shaft opening was approximately 56-feet, 8-inches above the next lower level. Employer is being cited under OSHA's multi-employer citation policy as the "exposing" employer.
