29 CFR 1910.147(c)(4)(i): Procedures were not developed, documented and utilized for the control of potentially hazardous energy when employees were engaged in activities covered by this section: On or about August 27, 2014, employees perform servicing and maintenance activities on equipment including, but not limited to, making changes to the spindles on the CNC machines, loading and removing parts on the Chuckers and other similar machines/equipment which have multiple energy sources including hydraulic, pneumatic, and electrical. Specific procedural steps for shutting down, isolating, blocking and securing machine or equipment to control hazardous energy were not developed, documented and utilized. Employees were exposed to the unexpected start-up or energization of the equipment. a. DP-2, employees working on the DP-2 are not utilizing control of hazardous energy procedures while performing servicing or maintenance on this machine. b. DP-4 (Davenport), employees working on the DP-4 are not utilizing control of hazardous energy procedures while performing servicing or maintenance on this machine. c. P-12 (Big Green), employees working on the P-12 are not utilizing control of hazardous energy procedures while performing servicing or maintenance on this machine. d. E-18, working on E-18 employees are not utilizing control of hazardous energy procedures while performing servicing or maintenance on this machine. e. E-23, working on E-23 employees is exposed are not utilizing control of hazardous energy procedures while performing servicing or maintenance on this machine. f. P-17, employees working on the P-17 procedures have not been developed, documented and utilized in the control of hazardous energy while performing servicing or maintenance on this machine. g. P-18, employees working on the P-17 procedures have not been developed, documented and utilized in the control of hazardous energy while performing servicing or maintenance on this machine. h. P-14, employees working on the P-17 procedures have not been developed, documented and utilized in the control of hazardous energy while performing servicing or maintenance on this machine. i. P-10, employees working on the P-10 were not utilizing hazardous energy control procedures while performing servicing or maintenance on this machine. j.
