29 CFR 1926.21(b)(5): Employee(s) required to handle or use flammable liquids, gases, or toxic materials were not instructed in the safe handling and use of these materials and made aware of the specific requirements contained in 29 CFR 1926, subparts D, F, and other applicable subparts of this part. (a) Georgetown Wastewater Treatment Lagoon: On August 28, 2014, employees entered a manhole (5' x 5' x 18.5') without receiving adequate instruction as to the hazard(s) and the precautions to take when using the Flex Seal sealant. Flex Seal contained flammable chemicals including toluene, parachlorobenzotrifluoride, acetone, heptane, isobutane, and propane. The Flex Seal was sprayed inside of a confined space that was approximately 387 cubic feet in air volume plus 75 cubic feet of water volume below grade. NOTE: Abatement certification and documentation are required for this item.
