29 CFR 1915.12(a)(1)(v): The employer did not ensure that spaces containing materials or residues of materials that create an oxygen-deficient atmosphere were visually inspected and tested by a competent person to determine the atmosphere's oxygen content prior to initial entry into the space by an employee. F/V Alpine Cove, St Hermon Harbor, Kodiak, AK: On or about 6 June 2014 and times prior thereto, the employer did not test for atmosphere's oxygen content while working in the engine room on a R-22 Freon refrigeration system. This condition exposed employees to an asphyxiation hazard from a R-22 Freon leak on the vessel.
