29 CFR 1910.132(a): Protective equipment was not used when necessary whenever hazards capable of causing injury and impairment were encountered: a) On or about June 12, 2014, and at times prior, employees were exposed to fall hazards while they set up carnival rides such as the Jolly Choo-Choo without adequate fall protection in place. Employees worked at heights of over 6 feet while standing on the top of the ride.
