29 CFR 1926.501(b)(4)(i): Each employee on walking/working surfaces was not protected from falling through holes (including skylights) more than 6 feet (1.8m) above lower levels, by personal fall arrest systems, covers, or guardrail systems erected around such holes: a) Employees were exposed to falling 10 feet through 4 ft wide floor holes in the plywood deck (top deck, 5th floor) during concrete formwork deck construction; the unprotected holes were left open for column forms to be lowered into the holes. Location: 81 Fleet Place, Brooklyn, N.Y; construction worksite. On or about 2/24/2014 NOTE: THE EMPLOYER IS REQUIRED TO SUBMIT ABATEMENT CERTIFICATION FOR THIS ITEM IN ACCORDANCE WITH 29 CFR 1903.19.
