29 CFR 1910.261(k)(18): The hazard of the nip points on all calendar rolls were not eliminated or minimized by means of an effective barrier device, or by feeding the paper into the rolls by means of a rope carrier, air jets, or hand feeding devices: a) On or about 12/3/2013, #3 paper machine calendar stack had the nip guards approximately 6" away from the ingoing nip points where threading was done. b) On or about 12/3/2013, #3 paper machine calendar stack did not have the aisle side access guarded near the floor. Opening approximately 22" wide and 5" high provided access to roll nips approximately 12" inside of bearing block. ABATEMENT CERTIFICATION REQUIRED
