Mattocks Five, Inc. dba Albion Mill, grain bin area- 73 Canal Street, Albion, Pa, 16401, on or about August 14, 2013, and times prior thereto: Company employees consumed food and beverages next to and around chemical containers. The chemicals included, but were not limited to, Hi-Yield 55% Malathion Insect Spray, and ProZap Formula 66.
