29 CFR 1910.252(a)(3)(i): Welding, cutting, or other hot work was performed on used drums, barrels, tanks, or other containers that had not been cleaned so thoroughly as to make absolutely certain there were no flammable materials present or any substances such as greases, tars, acids, or other materials which when subjected to heat, might produce flammable or toxic vapors: On or about July 24, 2013, employees were exposed to fire and explosion hazards at the terminal facility where water hauling tanker trucks were serviced and maintained. A cutting torch was used to remove bolts from the flange of a transfer valve that was connected to a tank that had not been purged of residues containing flammable vapors. The tank was used to transfer and haul liquid waste that also contained crude petroleum products.
