29 CFR 1910.134(c)(1)(ix): The written respiratory protection program did not contain procedures for regularly evaluating the effectiveness of the program: a) Embalming Room - On or about April 30, 2013, the employer's written respiratory protection program did not include specific criteria and information on how the Respirator Program Coordinator would annually evaluate the Respirator Program, the employees' respirator use, and the effectiveness of the respiratory protection equipment to protect employees from the hazards associated with airborne chemicals, including but not limited to formaldehyde, phenol, glutaraldehyde, ammonia, and sodium hypochlorite [bleach]. Abatement certification and documentation required within 10 days after abatement date. The certification shall include a statement that abatement is complete, the date and method of abatement, and state that employees and their representatives were informed of this abatement. Abatement documentation shall include documents demonstrating that abatement is complete, such as evidence of the purchase or repair of equipment, photographic or video evidence of abatement or other written records.
