29 CFR 1926.451(c)(2): Supported scaffold poles, legs, posts, frames, and uprights did not bear on base plates and mud sills or other adequate firm foundation. On or about March 13, 2013, a frame scaffold platform with a height of approximately 12 feet from the ground was released for use without legs bearing on base plates and mud sills or other adequate firm foundation. These condition exposed employees to fall and struck-by hazards due to unstable walking working platforms.
