29 CFR 1910.253(a)(4): Rules and instructions covering the operations and maintenance of oxygen or fuel gas supply equipment including generators, and oxygen or fuel gas distribution piping systems were not readily available: Employee(s) working in the shop using acetylene / oxygen welding equipment were exposed to fire and explosion hazards due to no rules or instruction being developed and implemented to inform employee(s) of the safe operation of welding equipment. A regulator was attached to an oxygen and acetylene cylinder that were being used for welding activities while in storage with other acetylene / oxygen cylinders.
