29 CFR 1910.119(c)(2): The employer did not consult with employees and their representatives on the conduct and development of process hazards analyses and on the development of the other elements of process safety management in this standard: a) Old Acid Room Area - The employer did not include operations employees and their representative for the 2002, 2006, and 2011 process hazard analysis (PHA) revalidations conducted on the Formaldehyde Loading System, on or about September 26, 2012. Abatement certification required within 10 days after abatement date. The certification shall include a statement that abatement is complete, date and method of abatement, and states employees and their representatives were informed of this abatement.
