29 CFR 1910.23(a)(2): Ladderway floor opening(s) or platform(s) were not provided with swinging gates or were not so offset that a person could not walk directly into the opening: Employees working on the elevated platform in the Wal-Mart area room were exposed to fall hazards of approximately 4' 1/2" from the working platform to the concrete floor. Provisions were not in place at the ladderway opening to keep employees from falling while standing and checking a gauge next to the access point. 29 CFR 1903.19(c)(1) requires certification that the abatement of the above violation is completed.
