29 CFR 1926.403(b)(1): Examinations of electrical equipment were not done to ensure the systems to be free from recognized hazards that were likely to cause death or serious physical harm to employees: a) On or about March 17, 2009 and at times prior Petrochem Insulation Inc. did not ensure that employees were protected from electrical hazards. The electrical system used for the turnaround demolition work was not inspected to assure the system to be free from recognizable hazards.
