Section 5(a)(1) of the Occupational Safety and Health Act of 1970: The employer did not furnish employment and a place of employment which were free from recognized hazards that were causing or likely to cause death or serious physical harm to employees in that employees were exposed to the hazard of crushing injuries due to structural deficiencies on cranes: a) Located on the 500AI Travelift, one fastener bolt head was broken off between the left rear column and rear top beam. b) Located on the 500AI Travelift, four of the fastener bolts between the left rear column and rear top beam were loose in that visual gaps were noted between the bottom of the bolt head and mounting plate. c) Located on the 500AI Travelift, the inside portion of the front top beam contained cracked welds on the bottom plate. d) Located on the 500AI Travelift, the inside top front beam was missing the trolley bumper. e) Located on the 500AI Travelift, the outside top weld between the trolley bean and plate had a cracked vertical weld. f) Located on the 500AI Travelift, one fastener bolt head was broken off between the front right column and right side beam. g) Located on the 500B Travelift, two fastener bolts were missing between the rear right column and rear top beam. h) Located on the 500B Travelift, two fastener were notably loose between the rear left column and rear top beam. i) Located on the 500B Travelift, three fastener bolts were missing between the left rear column and left side beam. j) Located on the 650AI (430-003) Travelift, two fastener bolts with missing heads were filled in with welding material between the right front column and front top beam. k) Located on the 650AI (430-003) Travelift, one of the fastener bolts located between the right front column and front top beam was physically loose in that it was capable of turning by hand. l) Located on the 650AI (430-003) between the left front column and front top beam, a bolt was missing on the traverse rope bearing cap. m) Located on the 650AI (430-003) Travelift, the trolley bumper was missing from the left inside front top beam. n) Located on the 650AI (430-003) Travelift, a cracked weld was noted in the rear upper trolley beam. o) Located on the 650AI (430-003) Travelift, the trolley bumper was damaged on the left hand side of the rear top beam. p) Located on the 650AI (430-003) Travelift, the trolley bumper was missing on the right hand side of the rear top beam. q) Located on the 650AI (430-003) Travelift, cracked welds were noted on the bottom plate of the rear top beam. r) Located on the 650AI (430-003) Travelift, one fastener was missing between the left rear column and the rear top beam. s) Located on the 650AI (430-003) Travelift, a vertical cracked weld was noted between the right rear column and rear top beam. t) Located on the 650AI (430-003) Travelift, one fastener was missing between the left front column and left side beam. u) Located on the 650AI (430-003) Travelift, one fastener was loose between the left front column and left side beam. v) Located on the 650AI (430-003) Travelift, two fasteners were missing between the right front column and right side beam. w) Located on the 650AI (430-003) Travelift, one fastener bolt was loose between the right front column and right side beam. x) Located on the 650AI (430-003) Travelift, one fastener bolt was loose between the left rear column and left side beam. y) Located on the 650AI (430-002) Travelift, one fastener bolt was broken off between the right front column and front top beam. z) Located on the 650AI (430-002) Travelift, the cable guide was broken off on the front top beam. aa) Located on the 650AI (430-002) Travelift, both inside trolley bumpers were missing on the front top beam. bb) Located on the 650AI (430-002) Travelift, the trolley stop located on the right hand side of the front top beam was broken off. cc) Located on the 650AI (430-002) Travelift, two trolley bumpers were missing from the rear top beam. dd) Located on the 650AI (430-002) Travelift, two fasteners were missing between the right front column and right side beam. Among other methods, one feasible and acceptable abatement method to correct these hazards is to: 1) Follow the Mi-Jack Travelift crane operators manual and do not operate cranes with missing or loose fasteners. 2) Follow the Mi-Jack Travelift crane operators manual and check for loose or missing fasteners daily or after every 10 hours of operation. 3) Conduct regular inspections of all Travelift cranes to identify cracked and broken welds.
