Section 5(a)(1) of the Occupational Safety and Health Act of 1970: The employer did not furnish employment and a place of employment which were free from recognized hazards that were causing or likely to cause death or serious physical harm to employees in that employees were exposed to the hazard of their foot slipping off the brake pedal of a cold header causing them to get caught in the ram section of the machine. (a) Back Room: Brake foot pedals on the cold headers were not provided with a non-slip contact surface. Abatement Note: Among other methods, one feasible and acceptable abatement method to correct this hazard is to apply a non-slip contact surface to each brake foot pedal on the cold headers as required by ANSI B11.7-1995.
