29 CFR 1910.178(l)(1)(i): The employer shall ensure that each powered industrial truck operator is competent to operate a powered industrial truck safely, as demonstrated by the successful completion of the training and evaluation specified in this paragraph (l). In a warehouse at the site, on or about March 12, 2012, an employee was moving welding machines, and particle boards with a powered industrial truck, the employee became trapped between a shelving unit and a powered industrial truck mast, exposing employee to caught-in between hazard.
