29 CFR 1910.178(l)(1)(ii): The employer did not ensure that each operator had successfully completed the training required by paragraph (l), except as permitted by paragraph (l)(5), prior to permitting an employee to operate a powered industrial vehicle. (a) On or about March 6, 2012, the employer failed to provide employees with training, which consisted of a class/instruction, written test, and operational test/evaluation, prior to allowing employees to operate the forklifts. This exposed employees to struck-by and crushing hazards.
