Text For Citation: 01 Item/Group: 002A Hazard:
29 CFR 1910.147(c)(4)(ii): Procedures did not clearly and specifically outline the scope, purpose, authorization, rules, and techniques to be utilized for the control of hazardous energy, and the means to enforce compliance including, but not limited to, 29 CFR 1910.147(c)(4)(ii)(A), (c)(4)(ii)(B), (c)(4)(ii)(C) and (c)(4)(ii)(D): a) Worksite - On or about April 14, 2025, employees were adjusting a butterfly valve by one employee stroking a butterfly valve (Fisher 6" 8580 300# RF Butterfly Valve) using air pressure while a second employee held the butterfly valve to stabilize it with their left hand while it was being lifted up by an overhead crane (R & M, Hoist Serial # 1315428, Hoist Model # SX40410050P35FBLOS). The second employee holding and stabilizing the butterfly valve was exposed to struck by and caught-in/between hazards when the the butterfly valve was stroked with air pressure and the employee's left thumb tip was caught between the disk and the edge of the butterfly valve. There were no energy control procedures for the adjustment of the butterfly valve that had air and mechanical energy sources.
