29 CFR 1910.178(l)(5): The employer did not evaluate the competency of the operator who had previously received training appropriate to the truck and working conditions encountered. On or about April 9, 2025, it was identified that employees performing recreational vehicle servicing and maintenance at BRV Lincoln, LLC located at 1000 and 1040 Saltillo Road, Roca, Nebraska were exposed to struck-by hazards in that the employer did not conduct an evaluation of all operators' performance prior to allowing operation. Instance include an operator with past undocumented experience from a previous company not having been given the hands on evaluation at this facility prior to operating the forklift.
