Shelving Department, Storflex Holdings, Inc., 392 West Pulteney St., Corning, NY 14830, on or about 1/22/2025; Lockout/tagout or alternate means of energy control were not developed, documented, or utilized by an employee who had to place their hand in the point of operation while unjamming equipment, including, but not limited to, the Basic Parts Line, "G Mechanica", ID # 1126. b.) Cooler Department, Storflex Holdings, Inc., 392 West Pulteney St., Corning, NY 14830, on or about 11/22/2024; Lockout/tagout energy control procedures were not developed, documented, or utilized by employees who performed servicing and maintenance tasks including, but not limited to, changing blades on the Upcut Vista Saw, Serial Number 014-0297.
