29 CFR 1926.1203(d): (d) If any employer decides that employees it directs will enter a permit space, that employer must have a written permit space program that complies with �1926.1204 implemented at the construction site. The written program must be made available prior to and during entry operations for inspection by employees and their authorized representatives. a) On or about October 30, 2024, at the AAC gas pit at the Cincinnati Zoo, John E. Carrigan Plumbing Co. failed to implement a written permit-required confined space program before directing employees to enter the confined space for the removal and replacement of gas lines. As a result, employees were exposed to a potential hazardous atmosphere, as the natural gas line had not been properly eliminated or isolated through engineering controls before entry, increasing the risk of an uncontrolled release of gas within the confined space.
