29 CFR 1910.147(c)(4)(i): Procedures were not developed, documented and utilized for the control of potentially hazardous energy when employees were engaged in activities covered by this section: Fenner Dunlop Americas, Inc. doing business as Fenner Dunlop ECS located at 5225 West Lakeshore Drive in Port Clinton, Ohio. On or about November 7, 2024, the employer did not ensure machine specific lockout tagout procedures were developed, documented and utilized for employees tasked with setup/changeover operations on the Siempelkamp Curing Presses. Employees were exposed to caught in and amputation hazards when engaged in setup/changeover procedures on curing presses #3 and #4 such as, but not limited to, pulling irons, securing irons, aligning fillers, and pulling a new mat through the curing deck.
