29 CFR 1910.147(c)(4)(i):Procedures were not utilized for the control of potentially hazardous energy when employees were engaged in activities covered by this section: a) Pour robot cage: On or about 09/30/2024, a lockout procedure was not utilized when the chemical pour robot nozzle detached as employee(s) attempted to tighten the nozzle. Employee(s) were injured as a result of being sprayed with a mixture of Toluene-2,4-Diisocyanate and hydraulic fluid. ABATEMENT CERTIFICATION REQUIRED
