29 CFR 1910.146(c)(5)(ii)(C): Before an employee entered a space, the internal atmosphere was not tested with a calibrated direct-reading instrument for oxygen, flammable gases and vapors and potential toxic air contaminants; On or about September 24, 2024, at the Cacique waste water pit, employees went inside a confine space without the required workplace hazard assessment being performed through a written certification, while performing work in a confined space.
