29 CFR 1910.147(c)(4)(i): Procedures were not developed, documented and utilized for the control of potentially hazardous energy when employees were engaged in activities covered by this section. a) Location - Press room area: Die maintenance on Press #139: On or about 05-10-24, a LOTO procedure was not utilized while unbolting and freeing the punch holder from the die. and the unexpected gravitational energy caused an unstable part of an upper die to fall and cause an injury to the employee's finger. b) Location - Short Run area: Die set-up on Press #424-49: On 07-11-24, LOTO procedures were not being utilized for die changing or die set-up tasks for punch presses. c) Location - Short Run area: Die set-up on Press #TN1-45: On 07-11-24, a LOTO procedure was not developed or utilized for die set-up tasks for a new power press.
