29 CFR 1910.147(c)(7)(i)(A): Authorized employee(s) did not receive training in the recognition of applicable hazardous energy sources, the type and magnitude of the energy available in the workplace, and the methods and means necessary for energy isolation. a) Production Area, Rip Saw Machine: On or about June 12, 2024, and at times prior, employees to electrical, struck-by and caught-in hazards, in that authorized employees were not instructed on lock-out/tag-out procedures and the prohibition to restarting or re-energize equipment, such as but not limited to the Rip Saw, that have been locked out.
