29 CFR 1926.502(d)(15): Anchorages used for attachment of personal fall arrest equipment shall be independent of any anchorage being used to support or suspend platforms and capable of supporting at least 5,000 pounds (22.2 kN) per employee attached, or shall be designed, installed, and used as identified in standards 1926.502(d)(15)(i)-1926.502(d)(15)(ii). a) On or about May 28, 2024, with respect to a worksite located at 2408/2410 Canter Lane, Green Bay, WI 54304, the employer did not ensure that roof anchor being used as part of fall protection system above the unit labeled 2408 had been installed correctly and would support at least 5,000 pounds per attached employee. There was not a sufficient amount of fasteners installed in the anchor point, as prescribed in the manufacturer's instructions. b) On or about May 28, 2024, with respect to a worksite located at 2412/2414 Canter Lane, Green Bay, WI 54304, the employer did not ensure that roof anchor being used as part of fall protection system above the unit labeled 2412 had been installed correctly and would support at least 5,000 pounds per attached employee. Two employees had lifelines attached to anchor point which would have increased the chance of an anchor failure during a fall emergency. c) On or about May 28, 2024, with respect to a worksite located at 2412/2414 Canter Lane, Green Bay, WI 54304, the employer did not ensure that roof anchor being used as part of fall protection system above the unit labeled 2414 had been installed correctly and would support at least 5,000 pounds per attached employee.
