Text For Citation: 01 Item/Group: 002A Hazard:
29 CFR 1910.178(a)(5): The powered industrial truck was equipped with front-end attachments other than factory installed attachments, however the employer did not request that the truck be marked to identify the attachments and show the approximate weight of the truck and attachment combination at maximum elevation with load laterally centered: (a)On or about May 13, 2024, and at times prior, the employer did not ensure the data plate on the Komatsu FG25 was updated or marked to show the changes in the overall lift capacity related to the use of two 70.5" fork extensions. The fork extensions were used to move larger material to include beverage counters. This condition exposed the employees to the hazard of improper forklift operation.
