29 CFR 1910.178(a)(4): Modifications and additions which affect capacity and safe operation of powered industrial truck were performed by the employer without the manufacturer's prior written approval: (a) 98 East Main Street, Leola, Pennsylvania - To reach elevated work, employees used two in-house made personnel platforms on the forks of any one of the following powered industrial trucks: Nissan forklift (model BGF03A45V, chassis number BGF03-920953), UniCarriers forklift (model MUG1F2A35LV, chassis number UG1F2-9L21014), or UniCarriers forklift (model MUG1F2A35LV, chassis number UG1F2-9L21246). The employer did not get the forklift manufacturers' prior written approval to use the attachments, which could result in hazards such as overloading the forklift or dropping a personnel platform and its occupants, on or about May 8, 2024. Abatement certification required within 10 days after abatement date. The certification shall include a statement that abatement is complete, date and method of abatement, and state employees and their representatives were informed of this abatement.
