Text For Citation: 01 Item/Group: 002A Hazard:
OSH ACT of 1970 Section (5)(a)(1): The employer did not furnish employment and a place of employment which were free from recognized hazards that were causing or likely to cause death or serious physical harm in that employees are exposed to crushing hazards from a load being dropped when operating cranes with hooks that had damaged throat latches: (a) Metal Fabrication Area: on or about and at times prior to April 24, 2024, employees operated 15/5-ton Shaw Box S/N 38783 to lift transformer housings weighing up to15-tons during manufacturing. The hook safety latch was bent upwards and did not make contact with the hook. Employees were exposed to the danger of being crushed-by steel transformer housings in the case of the load coming off of the hook unexpectedly. (b) Metal Fabrication Area: on or about and at times prior to April 24, 2024, employees operated 15-ton Cyclops S/N 1297 to lift transformer housings weighing up to 15-tons during manufacturing. The hook safety latch was damaged and did not make contact with the hook. Employees were exposed to the danger of being crushed-by steel transformer housings in the case of the load coming off of the hook unexpectedly.
