29 CFR 1910.23(b)(4): The employer did not ensure that ladder rungs, steps, and cleats that have a minimum clear width of 11.5 inches (29 cm) on portable ladders and 16 inches (41 cm) (measured before installation of ladder safety systems) for fixed ladders. a) On or about 04/15/2024 at the site located at 3000 Milton Ave, Solvay, NY 13209: Employees climbed a Werner Manhole ladder to insert a shovel at the bottom of the door for Kiln #10 exposing employees to a fall distance of approximately 3 feet. The ladder rungs on the Werner Manhole ladder were approximately 9.5 inches wide. b) On or about 04/15/2024 at the site located at 3000 Milton Ave, Solvay, NY 13209: Employees climbed Werner Manhole ladders to access the sand funnel for core machines #4 and #1 exposing employees to a fall distance of approximately 6 feet. The ladder rungs on the Werner Manhole ladders were approximately 9.5 inches wide.
