29 CFR 1926.501(b)(4)(ii):Each employee on a walking/working surface was not protected from tripping in or stepping into or through holes by covers: (a)Pro Framing, Inc. at 205 East 12th Street, Sioux Falls, SD 57104: On or about and at times prior to March 13, 2024, the employer did not ensure that a cover was in place on a sixth floor utility shaft while conducting roof sheathing operations. This condition exposed employees to fall hazards greater than six feet.
