29 CFR 1910.147(c)(4)(i): Procedures were not developed, documented and utilized for the control of potentially hazardous energy when employees were engaged in activities covered by this section: a)T & J Industries, Inc dba Metal Craft Industries at 1014 South Hillside Drive, Sioux Falls, SD 57110: On or about and at times prior to December 6, 2023, the employer failed to develop machine specific procedural steps for shutting down, isolating, blocking and securing machines or equipment with multiple energy sources. The Haas VF-3 Vertical Milling Machine, Serial 19258, was equipped with multiple energy sources and specific procedural steps were not developed to control hazardous energy. This condition exposed employees to the hazards of improper lockout. b)T & J Industries, Inc dba Metal Craft Industries at 1014 South Hillside Drive, Sioux Falls, SD 57110: On or about and at times prior to December 6, 2023, the employer failed to develop machine specific procedural steps for shutting down, isolating, blocking and securing machines or equipment with multiple energy sources. The Haas VF-4SS Vertical Milling Machine, Serial 1126830, was equipped with multiple energy sources and specific procedural steps were not developed to control hazardous energy. The Hennig Chip Conveyor KMO-0320-00, Serial 9760, was equipped with multiple energy sources and specific procedural steps were not developed to control hazardous energy. This condition exposed employees to the hazards of improper lockout. i)T & J Industries, Inc dba Metal Craft Industries at 1014 South Hillside Drive, Sioux Falls, SD 57110: On or about and at times prior to December 6, 2023, the employer failed to develop machine specific procedural steps for shutting down, isolating, blocking and securing machines or equipment with multiple energy sources. The Hennig Chip Conveyor KMM-1360-01, Serial 100874, was equipped with multiple energy sources and specific procedural steps were not developed to control hazardous energy. This condition exposed employees to the hazards of improper lockout. j)T & J Industries, Inc dba Metal Craft Industries at 1014 South Hillside Drive, Sioux Falls, SD 57110: On or about and at times prior to December 6, 2023, the employer failed to develop machine specific procedural steps for shutting down, isolating, blocking and securing machines or equipment with multiple energy sources. The Amada LC 3015 F-1 NT CNC laser cutter, Serial 100874, was equipped with multiple energy sources and specific procedural steps were not developed to control hazardous energy. This condition exposed employees to the hazards of improper lockout. Abatement Note: The procedures shall include but are not limited to the following: (1) A specific statement of the intended use of the procedures. (2) Specific procedural steps for shutting down, isolating, blocking and securing machines or equipment to control hazardous energy; (3) Specific procedural steps for the placement, removal and transfer of lockout devices or tagout devices and the responsibility for them; and (4) Specific requirements for testing a machine or equipment to determine and verify the effectiveness of lockout devices, tagout devices, and other energy control measures.
