29 CFR 1910.147(c)(4)(ii)(B): The energy control procedures did not clearly and specifically outline the steps for shutting down, isolating, blocking and securing machines or equipment to control hazardous energy: 38 Jackson Rd, Devens, MA: On and before October 24, 2023, employees utilized the Water For Injection 831 draft energy control procedure (ECP) that did not include all energy sources required to reach a zero energy state. A missing electrical energy source hazard was identified during a September 2023 preventive maintenance event, but was not added to the procedure until November 29, 2023. Additionally, this draft ECP did not identify or include steps to isolate the machine's thermal and pneumatic energy sources. On October 24, 2023, a Metrology Technician received burn injuries from uncontrolled pressurized steam thermal energy while removing a pressure transmitter from this machine.
