29 CFR 1910.28(b)(6)(i): (i) Each employee less than 4 feet (1.2 m) above dangerous equipment is protected from falling into or onto the dangerous equipment by a guardrail system or a travel restraint system, unless the equipment is covered or guarded to eliminate the hazard. a. On or about December 28, 2023, and at times prior, located in the Maintenance Dock at Door 100, the employer did not ensure that employees performing maintenance and servicing on the Haynes DUMPMASTER LD 6000 XD cardboard dumper at the uncovered and unguarded entrance to the Galbreath Compactor 1 (CMP 1) were protected from falling into the compactor. As a result, employees were exposed to falls into the dangerous equipment of the compactor below.
