29 CFR 1926.451(b)(2): Scaffold platforms and walkways were not at least 18 inches wide: On or about August 29, 2023, an employee was exposed to a fall of approximately 19 feet while performing siding activities on a residential home located on Lot 244, Victory Lane, Columbia Station, Ohio, 44028. The employee was working from a ladder pick scaffold that had an added section with a platform that was 12 inches wide. The employer did not ensure the platform was 18 inches wide.
