29 CFR 1910.178(a)(4): Modifications or additions which affect capacity and safe operation of powered industrial truck were performed by the employer without the manufacturer's prior written approval: (a) On or about July 13, 2023, five powered industrial trucks were used with additions in that two man baskets and two sets of forklift extensions (removable attachments) were used as needed and the employer did not have written approval from the manufacturers prior to allowing their use.
