29 CFR 1910.23(b)(9): The employer did not ensure that ladders are inspected before initial use in each work shift, and more frequently as necessary, to identify any visible defects that could cause employee injury. a) On or about July 11, 2023, where extension ladders, such as a 28-foot Werner ladder, were not inspected prior to employees using the ladders for tree trimming operations.
