29 CFR 1910.132(a): Protective equipment was not used when necessary whenever hazards capable of causing injury and impairment were encountered: a) At the worksite, 100 Bridlemere Avenue, Interlaken, NJ - front yard: Worker engaged in tree removal of a tree was exposed to fall hazards, up to and more than 50 feet to the ground surface below, when the worker was not secured to the tree with a personal fall arrest system. Occurred on or about June 14, 2023. NOTE: IN ADDITION TO ABATEMENT CERTIFICATION, THE EMPLOYER IS REQUIRED TO SUBMIT ABATEMENT DOCUMENTATION FOR THIS ITEM. FAILURE TO COMPLY WILL RESULT IN AN ADDITIONAL PENALTY OF $1000.00 IN ACCORDANCE WITH 29 CFR 1903.19.
