29 CFR 1910.119(f)(1)(iii)(E): The employer did not develop and implement written operating procedures that provided clear instructions for safely conducting activities involved in each covered process consistent with the process safety information and did not address any special or unique hazards: At the facility located a 3019 Scanlan Avenue, Salina, Kansas, the employer failed to include in the standard operating procedures for the Evaporative Condensers the cable locked valves as a critical safety valve. Employees and contractor employees using the Evaporative Condenser standard operating procedures for the ammonia refrigeration system were not provided with a description of the safety critical valves being included in the procedure. Employees engaged in operating, inspecting, testing, and maintaining activities in the ammonia refrigeration system at the facility were exposed to catastrophic release of ammonia which results in asphyxiation, explosion and flash fire leading to death and injury.
