29 CFR 1926.1412(d)(1): A competent person did not begin a visual inspection of the crane prior to each shift the equipment was used or did not complete it before or during that shift: (a)Henry Carlson Construction, LLC at 1508 West 22nd Street, Sioux Falls, SD 57105: On or about and at times prior to May 25, 2023, the controlling employer did not ensure the operator of the Liebherr 550 HC40 20-ton tower crane conducted a visual inspection of the crane prior to the shift or completed it during the shift. This condition exposed employees and subcontractor employees to struck-by hazards.
