29 CFR 1910.146(d)(3): Under the permit-required confined space program required by 29 CFR 1910.146(c)(4), the employer did not develop and implement the means, procedures, and practices necessary for safe permit space entry operations, including, but not limited to, the requirements in 29CFR 1910.146(d)(3)(i)-(iv): a. On or about September 2, 2023, an employee entered spiral freezer 7, a permit required confined space, the employer did not implement the means, procedures, and practices necessary for safe entry. b. On or about September 3, 2023, an employee entered spiral freezer 7, a permit required confined space, the employer did not implement the means, procedures, and practices necessary for safe entry. c. On or about September 10, 2023 an employee entered spiral freezer 3, a permit required confined space, the employer did not implement the means, procedures, and practices necessary for safe entry. Employees were exposed to hazards related to anhydrous ammonia. Abatement documentation is required for this item in accordance with the requirements of 29 CFR 1903.19(d).
